How do I create a Feedback QR code?

Click Create new, then Create a QR Code, click More on the Type step and choose Feedback. Name is the only required field; Title is the question at the top of the page, and star ratings plus a comment box are switched on already. Feedback codes are dynamic only, and every response is collected under Data Hub in the dashboard.

Here is how to create a Feedback QR code. One scan opens a page with a rating and a comment box. No app to install.
Use it on the card in a hotel room, on the bill at a restaurant table, or on the card a service van leaves behind. Because the code is dynamic, you can change the questions later and the printed code keeps working.
In the dashboard, click Create new, then Create a QR Code. Pick Feedback from the type grid and leave it on Dynamic. Give the code a name, then type the business name, the title question, and your website. Choose a theme, then Download. The code is saved and ready to print.
Three things matter. First, the name and the title. The name is the only field you must fill. Second, the assessment. Star ratings or yes and no, plus a comment box. Third, the categories. They are optional. Add rooms or breakfast to rate each one.

When to use a Feedback QR code
- Hotel rooms and checkout desks. A card on the desk catches the comment guests would not send by email.
- Restaurant tables and bills. Ask while the meal is fresh, not a week later.
- Service vans and delivery notes. The customer rates the visit the moment it ends.
To send people to a public listing instead of a private form, use a Google review QR code. The feedback QR code generator runs the same wizard.
Steps in the dashboard
- Open the wizard. In the dashboard, click Create new, then Create a QR Code.
- Pick the type. On the Type step the grid shows the first 15 types, so click More to show the rest and choose Feedback. This type is dynamic only, so the Dynamic tab is already selected and Static is unavailable.
- Fill in the content. Click Next →. The first field is QR Code Name — it is optional and only for your own list. Then, under Basic Information:
Field What it does Name Required. Your business name, shown at the top. Title The question people answer, for example Leave us your feedback. Description Optional line under the title. Assessment Ratings (stars) or Answer Yes / No, plus Comments. Both on by default. Subcategories Optional. Add rooms, breakfast or service so each is rated separately. Redirect URL (optional) Where to send people once they submit. Mandatory email and Mandatory phone switches, and Enable sending reviews by email with an Email address, are all optional.
- Design and download. Click Next → to reach QR Design, pick a theme or set your own colours, then click Download ✓. The code is saved to My QR Codes at the same time.
Where the responses go
Open Data Hub in the dashboard sidebar. It lists every code that collects responses, with Data-collecting codes, Responses captured and Awaiting responses across the top. Open a code to read the answers, see the average rating roll up, and export them with the CSV button.
Each response is stored with the time and the country or city it came from. Scans are recorded separately, in Full analytics.
Common problems
The code does not appear in Data Hub
Only types that collect responses are listed, and Feedback is the only one today. If yours is missing, check you picked Feedback rather than Review.
Nobody leaves a comment
Check Comments is still switched on under Assessment, and keep Mandatory email off — asking for contact details first cuts responses.
People land back on a blank page
That is Redirect URL (optional) pointing somewhere unexpected. Clear it to leave people on the thank-you screen instead.

Do people need an app to leave feedback?
No. The rating page opens in the phone's browser.
Can I change the questions after printing?
Yes. Feedback codes are dynamic, so edits reach every printed code.
Can I export the responses?
Yes. Open the code in Data Hub and click CSV.
Is Feedback available as a static code?
No. It is dynamic only, because the page and the responses are hosted.
