Read down the columns and the conclusion is hard to avoid: there is no row where one option wins everything. The chip is the only thing that survives a lost collar. If they decline, or the prompt times out, you still get an approximate area derived from their network connection. Both are recorded, and you can tell which is which.
During a search this changes the questions you can answer. A scan twenty minutes ago means somebody has your dog right now and is looking at your phone number — you are not searching any more, you are waiting for a call, and that is a completely different afternoon. Write it for a stranger in a hurry, holding your pet, reading on their own phone. Not for yourself.
A phone number, then a second one. The first is obvious. The second matters more than people expect — you will be in a meeting, on a flight, or out of battery at exactly the wrong time. A partner, flatmate or parent as backup roughly doubles the chance someone picks up.

Making a tag that survives a collar
A QR code on a pet tag lives a harder life than almost any other printed code: rain, mud, sand, other dogs' teeth, and a few thousand hours of scraping against a metal ring. A few things make the difference between a tag that still works in three years and one that quietly stopped working last spring.
Use a dynamic code. With a static code the details are baked into the pattern and can never be changed, so a new phone number leaves the tag pointing at nothing.
